Goals & Pagetracker

A while back, I setup a pagetracker to track the clicks on our contact
forms. We have 3 different forms and I was having them submit to a
container /form/contact .

I decided I wanted to switch to goals to track a head match for when
the form is submitted.

Ever since then, two of the forms track in goals, but the other does
not. I've tried setting this up on new accounts under new domains,
but no luck.

My question is, does anybody know why this might be happening, and
should is stick with using page tracker to track the form submissions?
